Some men report success with VCDs (vacuum constriction devises) that manually bring blood to the penis in order to maintain erection. The device can be used for up 30 minutes, though some bruising or swelling may occur. In addition, the erection achieved does not have the same appearance nor sensation as a natural erection, though ejaculation and orgasm remain unchanged.

Some men choose to have a medical procedure in which surgeons implant a penile prosthesis, which can either be malleable or inflatable. The simplest device causes the penis to be in a state of constant semi – erection; lifting the penis up slightly causes a full erection. Many men opt for an inflatable or hydraulic prosthesis, which looks much more natural cosmetically.

The implants tend to only be recommended if all other methods, both natural and conventionally medical, have failed. A man who has the procedure done but later decides to have the prosthesis removed may never again be able to sustain a natural erection, as some of the sensitive nerves in the penis become irreparably damaged during the procedure.

Another surgical procedure generally reserved for young men who have sustained some sort of blunt trauma or damage to the pelvic area (specifically affecting the blood vessels of the penis) is vascular reconstructive surgery. The surgery generally involves bypassing blocked arteries by transferring an artery located in the abdominal muscle down to the penile artery. This creates a path of blood flow which can evade the blocked artery that has inhibited proper blood flow to the penis and thus negatively affected erections prior to surgery.

In addition to being quite invasive, the success rate for this particular surgery as a cure for erectile dysfunction has been somewhat disappointing overall, though for the specific target group (young men with a very specific type of injury, often sports-related), the success rate falls between 50 and 75%.
